  • 24 Hour Care
    Providing ongoing support and assistance to someone throughout the entire day and night, every day of the week. It’s designed for individuals who need constant attention due to medical conditions, post-surgery recovery, or complex care needs. Caregivers are available at all times, including overnight, to provide support and supervision
  • Home Doctor Evaluations that come to you
    Understand your overall health and the steps you can take to prevent or manage conditions. We offer in-home and virtual visits, including in-home screenings for chronic conditions. The Home Doctor provides comprehensive primary care and specialty medicine to residential care.
  • Transitional Care Management (TCM) 
    Focuses on supporting patients as they transition from acute care settings (like hospitals) to community settings like their homes  To prevent unnecessary hospital re-admissions and improve patient outcomes after discharge from acute care setting .TCM can help patients manage their health conditions better, improve their quality of life, and reduce complications after discharge. 
  • Memory Care
    • Secure Environment: Designed to protect residents from wandering and self-harm. 
    • Specialized Staff: Trained in dementia care, providing individualized attention and support. 
    • Personalized Care Plans: Tailored to each resident’s unique needs and the progression of their memory loss.

  • END OF LIFE AT HOME  (HOSPICE)
    Involves providing comprehensive support to individuals and their families during the final stages of life, focusing on improving quality of life and comfort. This can include managing symptoms like pain, shortness of breath, and anxiety, as well as addressing emotional, spiritual, and practical needs. Home-based palliative care and hospice care are common options for end-of-life care at home. 
  • Diabetes Care
    • Healthy eating: A balanced diet that emphasizes fruits, vegetables, whole grains, and lean protein helps regulate blood sugar levels. Portion control and limiting added sugars and processed foods are also crucial. 
    • Regular exercise: Physical activity improves insulin sensitivity and helps maintain a healthy weight, both of which are important for managing diabetes. Aim for at least 150 minutes of moderate-intensity exercise per week. 
    • Medication: Depending on the type of diabetes and individual needs, medication may be necessary to control blood sugar levels. This could include insulin injections, oral medications, or other therapies. 
    • Blood sugar monitoring: Regularly checking blood sugar levels helps individuals understand how different foods, activities, and medications affect their blood glucose levels.
